After lots of futzing with confusing BIOS screens, I found the menu option to
enable bus mastering on the PCI Slot I had my PVR 250 installed on. For some
reason 'Master' was set to 'Off'. I turned it 'On' and rebooted, and voila, my
card got detected:

Nov 1 03:00:18 localhost kernel: ivtv: Enabling pci device
Nov 1 03:00:18 localhost kernel: PCI: Found IRQ 10 for device 00:0e.0
Nov 1 03:00:18 localhost kernel: ivtv: Bus Mastering Enabled.ivtv: 2051 (rev
1) at 00:0e.0, ivtv:      irq: 10, latency: 32, memory: 0xe8000000
Nov 1 03:00:18 localhost kernel: ivtv: attempting ioremap at 0xe8000000 len
0x02000000
Nov 1 03:00:18 localhost kernel: ivtv: attempting ioremap at 0xea000000 len
0x00010000
<Many more lines of output>

And what's even better I finally got lirc to work as well. Thanks everyone!
Just a note - if you have an older motherboard like me (mine is from '99) you
need to make sure BusMaster'ing is enabled on the slot you installed your
PVR250 card. This is done from the BIOS!
