I/O Magic PC PVR card; something like $5 after rebate from CompUSA. 
Stereo output jack, but the same levels on both channels.  I/O Magic
Tech support forum that couldn't be convinced of the difference
between a true stereo signal and mono x 2 signal.

But, hey, $5.
