This was typically the case if there was a problem opening the 
destination file for writing (directory doesn't exist; directory exists 
but isn't writable; file already exists and isn't writable; filename 
has problematic chars in it (this happens to me because my music 
filesystem is VFAT)).  Some of the issues have been addressed by a 
patch recently, IIRC, but you'll want to examine your music ripping 
setup and make sure your directories & permissions are correct.  Also, 
look at the terminal output from mythfrontend when this happens; it 
will likely tell you exactly which file it's dying on.
