Try turning off artsd, conveniently located in the KDE control panel (in the
sound settings under sound & multimedia, not on the general services control
applet). I was in the same boat as you. I upgraded my system (RH9, ATrpms,
Leadtek TV, Intel 845 for sound) to 0.11. All was fine. Then I added the
DVD module and lost all sound. After some random screwing around, it came
back. The mixer (kmix) settings always seemed to have a mind of their own.
I turned of artsd (drill into the KDE control panel), reset settings in
kmix, burned incense, and rebooted a few times. It's not scientific, but
I'm back in business.
