[mythtv] MediaMonitor & DVD-drive speed limiting

Nigel and others.. if I looked it right, it seems that DVD-drive speed 
limiting is called only when MediaMonitor is turned on. Is this 
intentional and is changing it a feature request or a bug? :) I think 
speed limit should/could be called always regardless of the MediaMonitor.

If these really are related, it would be more clear to put DVD-speed 
limiting settings "under" MediaMonitor in setup so that it is clear that 
MediaMonitor is needed for speed limiting.
