...but remember that the GNU GPL  *DOES* entitle you use MythTV for
commercial use.   So, please feel free to go ahead, on the condition that
you make available your changes back, as stated in the GPL.

Yes you can build a settop box, or whaever you like, just release your code
changes, see the GPL.
It's really very easy, and the most likely things to occur are:
1) developers will ignore your changes as they are not good-enough for them
2) developers will love your changes, and fix bugs in them for free that you
completely missed.
3) some combination of the above.
