[mythtv] HOWTO: make Juski's trunk themes work on 0.20.2

Hi,

Juski was kind enough to let me know how to use this themes which can be
found in SVN in 0.20.2:

15:04 <juski> remove the widescreen, dolby, subtitle (not CC) and mono
icon definitions
15:05 <juski> that's it!


Just a FYI. I probably should have posted this on -users, but I'm not
subscribed there. Maybe someone can forward this and/or post some patches.
