[mythtv] Plugin Guide: Hello Myth

I'm new to creating plugins and have created a simple MythHello plugin 
that demonstrates how to build a simple skeleton plugin.  It doesn't do 
anything, but shows a simple UI page with a title.  I've posted this 
example to the wiki: http://mythtv.org/wiki/index.php/BuildPlugin_HelloMyth

I'd love to see people expand on this guide by adding buttons and some 
functionality -- and correcting any of my mistakes.
