[mythtv] Ticket #4270: MythTV on Windows - now with comprehensive documentation!

All,
Richard Dale and myself have just spent a few hours revising/improving the
documentation for building mythtv on windows:

(thanks to Simon and Zack for help along the way too)

It's now really well laid out, and should be considered the "definitive"
documentation for Win32 builds untill we have a "build script" like OSX does
to do all this for us.... although it's still likely that you will have
issues with this process, there is a much greater chance of success now!.

PS. Andrei, as the "man" for Win32 builds,  the editors would especially
appreciete your feedback/comments.
