[mythtv] Update of contrib/mythname.pl

[Behan Webster]

Attached is an update for mythname.pl (in contrib) which works with the
newer database schema.

I've added a few more features as well.

Should I be submitting this as a trac issue to ask to have it included
in the svn repository?
