[mythtv] mythtranscode "missing" cutpoints

If you run mythtranscode with the "-v general" command line option, it
should print out a message like "Fast-Forwarding from 1242 to 1850" every
time it skips a cut portion.

You could also try putting some debug statements in
NuppelVideoPlayer::TranscodeGetNextFrame() to see when the cuts are skipped
or not skipped and maybe that would give a clue as to why.
