[mythtv] [PATCH] DVB Scanwiz update.

This patch tidies up the scanwizard code. I've also included the into
patch the following as well (mainly because it was getting hard to keep
things separate :) :

the dvbsignalmonitor patch which seems to cure Ed Wildgooses issues.
the fix for the scanner to reach the end of the mux list
eit program duplicate detection
more uk eit guide fix ups
changing of delete to delete[] in dvbrecorder (Tj's patch)

There is also separate patch to remove cruft I'd introduced pre 0.17
into settings.*
