[mythtv] [PATCH] Mac OS X video: more goodies

This new version uses the same premise as my previous implementation 
(using QuickTime to decompress and scale the video), but it has a 
cleaner and more flexible architecture, which enabled me to add several 
bells and whistles:

- Video can be displayed fullscreen, in the main window, in the dock 
tile, in a floating window, or on the desktop.

- You can set a frame-rate divider, to get better performance for your 
machine.

- The main or floating windows can be translucent.

There are a few bugs remaining, but I haven't been able to make much 
headway on them.  See videoout_quartz.cpp for a more detailed list, and 
for the various compile flags to turn on the goodies mentioned above.

This will probably be the last patch from me for a few weeks, as I need 
to catch up at my day job before a vacation next week.  In the 
meantime, please report any bugs or send me backtraces.  Nigel, if I've 
left out anything important from v1, please let me know or submit a new 
patch; if this is stable enough, I think it's ready for CVS.
