[mythtv] PATCH Potential DataDirect icon solution

Fixed:

- a minor bug (document tag still had DD remnants).
- zap2it is no longer put into your settings table (an import of a proper
map will have any needed URL bases).

Now attaching na_icons_to_icon_map.pl which will automate constructing a map
for those who choose to (it makes a nice starting point if you want to make
a real map as well).

Unlike the last time I submitted my patch this one really is the last one
unless someone finds some bugs.

If applied the following should build a non-master map and grab all
available icons for your lineup (assuming you have tv_grab_na_icons
working):

perl na_icons_to_icon_map.pl --grab --share=/tmp --out=myicons.xml
mythfilldatabase --import-icon-map myicons.xml --update-icon-map
