I emailed d0m today to ask, but that appears to be what he did except he stripped the X server and is manipulating the framebuffer directly.  This is exactly what I want to do and have been investigating.

I'm going to play with a stock VNC server tonight and see what I can do with it.

I think once the OSD is working, the streaming part will be easy.  Famous last words.
