[mythtv] mythmusic patch to use arts sound server

Here is a patch against current CVS that will let mythmusic work with
arts (a sound server that kde uses).

Run configure with --enable-arts to enable arts support.

For the moment it is done with ifdefs since mythmusic doesn't support
multiple output devices like mythtv does and because there is a lot of
integration in the output source file that shouldn't be duplicated into
another output file.

I'll send along the mythtv patch shortly.  No more having to shutdown 
artsd to run myth!

Let me know if I need to change anything.
