[mythtv] [patch] XvMCSurfaceTypes cleanup + print

This creates an XvMCSurfaceTypes.cpp file and moves the more major
functions from XvMCSurfaceTypes.h there. It also adds a couple print
methods and a ostream&<<(ostream&) method for easier debugging.

This was part of the XvMC efficiency patch, which I'm breaking up into
easier to apply chunks as promised.
