[mythtv] [PATCH] thumbnail preview generation bug

This should solve a longstanding bug:

We were assuming Encoder 1 is local. If this doesn't happen we enter
uncovered states, and it just retries never generating a thumbnail. Also we
weren't returning a proper response for not enough/bad data. This one
probably doesn't fix anything right now, but it should keep things from
breaking later.
