[mythtv] 2 tv cards and selecting the correct /dev/dsp device insetup

Just to let you know... The new setup (on my RedHat 8 boxes) segfaults
if you don't have root access. I've verified it on two machines.

I'll go one better. ;-)

The selection lists now scan for all device nodes and include them in
the
list, so there are no more fixed limits.
