[mythtv] PATCH TV within mythmusic

Attached is a patch that enables watching LiveTV with no sound while in
mythmusic.

To access the functionality you must have the KeyboardAccelerators
enabled in your configuration.  The letter M (typically mapped to the
EPG) brings up the TV within the main mythmusic screen.

Visualizations are disabled while watching TV and the TV functionality
is not available if a visualization is displayed.  Otherwise the
visualizations are left alone.

A small change is made within the tv_play files to allow creating the TV
class with disabled audio.  (So you will have to recompile MC prior to
mythmusic) to use it.
