             MythPlayer: Remove old a/v sync code and improve new

 - remove the setting for AVSync2 and its associated sync adjustment
 setting
 - remove old AVSync method and rename AVSync2
 - the sync adjustment is replaced with new code from Tim Pletcher (with
 thanks) to smooth and dampen asjustments.
 - in testing, playback is smooth, initial a/v sync is better and there
 is no need for the extra sync adjustment setting on platforms that
 return erratic audio timestamps (e.g. Raspberry Pi).
