 Fixes for MythUI settings

 - New Edit / Delete menu that supports all settings.
 - Remove obsolete unused channelgroupsettings.cpp and
 channelgroupsettings.h
 - Remove non-working unneeded Add / Delete from Recording Profile Groups.
 - Remove now superfluous class VideoSourceDialog
 - Fixed many MythUI Setup bugs.
