 The index isn't used, as everything that matches *.php ends up getting
 passed through to php-fpm and essentially everything gets rewritten to
 mythweb.php something. I can change it if there is a better pattern than
 what I am currently doing.

 As far as passing the hostname I think you are correct that it is not
 required. I'll check later and update the pull request.
