HTML Setup: Channel editor improvements and Mux API fix.

* Multiplex lister API: Make sure to limit DB query by source ID.

* Channel editor.  In the advanced pane, present a dropdown list of configured muxes so that if a channel moves to a known transport of frequency, it's as simple as picking it from the dropdown and adjusting the service ID.

Use HTML 5 "number" input types where appropriate.
