HTML Setup: Various updates.

* Add a edit screen for video sources.  This is far from what the final product should look like.  Realistically, each type of source should be a different class with widgets hidden by default and only shown if they are relevant to the listings type. But this puts something out there and I can start adding the relevant CSS classes and switching to easier to use widget types.

* Add context (right click) menus to the channel and guide editor to allow one to quickly edit or delete items on mouse over.
