 (In [16134]) Fixes #4698. Misc bug fixes for MS Win video output.

  * fixes embedding
  * fixes fill/zoom modes
  * fixes fullscreen mode

 Note: I removed the text reformatting portions of the patch by Andrei.
 Those should be a separate ticket anyway + we probably want to wait on
 that until after the 0.21 so that the branch and trunk don't diverge due
 to simple whitespace changes...
