 Attached is a revised $PREFIX/share/mythtv/setup.xml patch  and also a new
 patch for Minimalist-wide.
 Minimalist-wide is the only official theme that is affected by the
 setup.xml change, all the other themes already had support.
 I'm guessing this a throw back to the classic menu scheme.
