Log:

Re-write the screen shot grabbing. Should be a lot more efficient
now because it only uses native avcodec functions and doesn't use 
the NVP to grab screen shots so there is no longer any need to create
a markup map before grabbing the images. Also fixes a problem where
occasionally the first 3 or 4 chapter point images would be grabbed OK 
but later images would all end up being the same image from the middle 
of the file.
