 I was also plagued by this problem.  I can't be sure I wasn't twiddling
 other options as well, but it seems that changing to "Standard" decoding
 *and* turning OpenGL vertical sync OFF has made this issue completely
 disappear for me.  Perhaps that's where part of the problem lies...?

 That's with a GeForce 5200 and the "stable" 9-series Nvidia drivers.
