Mac OS X fixes from Eggert Thorlacius.
 Commented out a CFRelease() which appears to cause problems in DisplayResOSX
 Moved videoout_xv.h include in videobuffers.h into USING_XVMC.
  (I also removed the unneeded xvmc_render.h include from videobuffers.h.)
