Log Message:
library rework.  Make libmythtv, libavcodec, and libavformat shared libs.
Move libvbitext and libdvbdev into libmythtv.  Cuts 20MB off the installed size
of the binaries in Release mode, increases make and make install speed, etc.

Bump up the library versions to 0.14.

Probably need to do a make distclean.

Might break DVB compilation, though I tried not to.  Include the DVB compilation
patches from Martin Marshall and John Pullan.
