cle266 decoder fixes from Ivor Hewitt.
Ability to compile without Xv, patch from Wayne Hogue.
Remove the first usleep from RemoteFile::Reset, shouldn't be needed anymore.
usleep -> qwaitcondition patch from Steve Brown.  Seems to work ok.
