I had the same issue with PVR-250 and my DirectTV.<br>

You need to execute (and I think I remember the right path here)
/usr/lib/ivtv/ptune.pl (or, for a little gui, ptune-ui.pl).<br>

For me, setting input to 'Video 0', and then selecting 'channel 3', and
then hitting '3' worked.<br>

are you trying to record from cable or broadcast tv? have you tuned to
the desired channel with the ptune.pl script? if you're using cable and
the standard cable frequencies don't work, try the cable-hrc set. it
seems that a good deal of the u.s. uses this standard.
