ir-keytable -v -c -p nec -a <full path to tivo34_nec>

Assuming you only have (1) IR device plugged in, you do not need to specify
device.  If you do have multiple, use -s ir{0,1,2}

It's all in the man page

-v should tell you what is wrong.
