Issue #3 - Well, I feel like a FOOL!  Following the advice here:

I set the resolution in /boot/config.txt as stated:

hdmi_drive=2 # Uncomment this hdmi_drive line if using sound via HDMI
cable
hdmi_group=2  # DMT
hdmi_mode=39  # 1360x768 60 Hz
----- end lines -----

And that fixed my video issue - I didn't even have to set the GUI
settings.  It looks perfect.

Issue #2 - Once I hit M (rescan) everything was fixed.  No issues.

HOWEVER - I still don't get why the MPEG2 key causes the video to be
worse.  I just disabled it - but I would like to use the key if I can. 
Any tips on what I am doing wrong?  Other than that, I'm ready for prime
time on this, now.  I can swap the old machine out, make a copy of the
SD card, get a spare RPI3 in case of an issue and I am set.  I am SO
excited that this works!  Thank you to the developers for making this
happen - a $35 (well, $50 for the case/ps/etc) front end that consumes
hardly any power.  The wife factor is super high on this, too!
