I have noticed this when there are generic episodes (i.e. Episodes that
have no specific description). My solution is to select "Identifiable
episodes only", which prevents the generic episodes from recording.
Generic episodes are normally reruns. You may actually want those
generic episodes recorded, in which case you probably need a custom rule
that selects a particular channel.

I also notice that when trying to record the news, this problem
invariably happens, it wants to record the same news show off many
channels, including record it off the HD and SD copies of the same
channel. I solve it by selecting "this channel" or creating a custom
rule that uses channel id to select which one to record. This is not
optimal but it works for me.
