I think you're right on both count.

The issue may be the SSD and/or the BTRFS file system.
I found a fair number of errors on it.

I will upgrade to 16.04 as it includes a newer version of btrfs.
14.04 comes with 3.3, which isn't recommended to fix errors either :(
