For me, the occasional miss was being caused by incorrect programid data from the listings. Only occasionally. I got around it by using keyword recording rules for all my rules now. Just watch that a keyword rule for "Q & A" won't match "Q and A" or "Q&A".

Some of your example title changes probably aligned with a seriesid/programid change anyway. Ie the series "Masterpiece" is probably a completely different series with completely different programid's to "Masterpiece Classic".
