I installed the required driver (mental note to add it to the requirements
for the RPM Fusion mythtv package) so it's actually using it but the result
is more or less the same. Lot's of "Video is XXXX frames ahead of audio"
and buffer underruns. Something has obviously changed but I'm no closer to
figuring out what.

I even upgraded from Fedora 20 to 21 just to see if a newer kernel or
library would "fix" it. A long shot but figured it was worth a try.
