I would have said something much earlier if I knew this was your problem.
Before I made the changes on the wiki, mythfilldatabase was taking a very
long time.  After making those changes, there was a huge difference.

I created /etc/my.cnf.d/mythtv.cnf with the changes listed above and this
seems to have fixed things.  After that, I ran the mysqltuner.pl script and
made the suggested changes.
