Was the box off for a while? It might be possible the card is
overheating? I don't recall if you said your card has a fan or not. If
so, is the fan turning? Is the case getting enough airflow? Is the box
near a heating vent? Sorry if these sound basic, I'm just trying to
cover the bases.
Another thought is and if you are comfortable in doing it, is the heat
sink on the card easily removable? If so, you can try removing the
heatsink, cleaning the thermal paste off of the sink and chip and
applying fresh thermal paste and reattaching the heatsink. Sometimes the
paste will dry out and reapplying it allows the heatsink to dissipate
heat properly.
