If you have multiple monitors on a MythTV system (even if you don't use 
both--for example, some laptops enable both the built-in monitor and the 
external display output, even when the external display isn't 
connected), you MUST specify the appropriate monitor information in 
MythTV Appearance settings:

Display on screen
Run on the specified screen or spanning all screens.

Monitor aspect ratio
- 16:9 - 16:10
- 4:3
The aspect ratio of a Xinerama display cannot be queried from the 
display, so it must be specified.

and do NOT specify:

Video aspect override
When enabled, these will override the aspect ratio specified by any 
broadcaster for all video streams.

for any reason, even if you think that's the only way to make your setup 
work properly.  If you specify "Video aspect override" and it seems to 
make things work, it simply means you haven't properly configured your 
system (and it will cause some video to display incorrectly)--so fix the 
misconfiguration rather than use the broken-idea "all my video was 
encoded by an idiot who didn't set the proper aspect ratio, so ignore 
what the video says, and I promise to never, ever, ever watch a video 
with a properly-specified aspect ratio that is different from what I've 
said every single video I will ever watch will be" setting.
