Further to my previous post about TV power cycling and subsequent refresh rate issues, I've gone back and re-tried some of the basics that hadn't worked for me in the past, and now found a setup that works. The solution was simply to leave xfsettingsd not running (to fix the TV power cycling problem), and to enable “Separate video modes for GUI and TV playback” in the MythTV Frontend setup (setting the resolution to 1920x1080 and leaving the refresh rate on "auto") to fix the refresh rate problem.
