I created the KEY_BLUE line at the end by copying one of the others and
then changing the button and config attributes. Originally I had sections
for KEY_RED, KEY_YELLOW, and KEY_GREEN, but I removed those for now so I
can focus my efforts on just getting the blue button working. Obivously, I
do not expect the RED, GREEN, or YELLOW buttons to work until I add their
mappings for those back into the file.

I should also mention that I have restarted the lirc service and the
mythfrontend process after making any changes. Maybe that is unnecessary or
maybe there is something else I need to do too.
