You can order one from Hauppauge by calling them.  If they're nice enough,
they might even send you one for free if you indicate yours failed.

They are a 5v, 2amp adapter - you can also find them online.  They actually
sold some with a 3amp adapter... but, the vast majority were sent with a
2amp adapter, including the replacements.
