In the real world.
There are many threads to existence.
People have different ideas, which is *really* cool.

Also in the real world for me, my father suffers from moderately
advanced dementia. I'm an quite literally beyond bankrupt supporting
him. Now, we *all* live on his pension. Some three people must starve to
death to achieve the financial obligations.
As there are precisely three people present, that is a no-brainer.

Here is the ugly truth that you object to.
My father is 83 years old. He started to ignore his conditions when my
mother died shortly after 9-11. I came home after military retirement
after I was informed that his condition deteriorated.
It was worse than reported.
Still, when I repatriated, I was sworn by my father to let him die in
his own bed.
Idiot me, I accepted, as did my wife.
Every day I get to see little of my father, but my sworn word.
Every day, I'm torn between shooting my mouth, shooting my brain stem
and putting my frontal lobe out to pasture.
