Actually, the upgrade should occur even in the event of a DB backup 
failure, assuming the user says to continue after the popup that says, 
"This cannot be un-done, so having a database backup would be a good 
idea," instead of choosing to exit.  That said, we recommend users never 
upgrade the database without a backup--so if the automatic backup 
creation fails, you should only upgrade if you manually created a backup 
yourself.

Though I'll admit I didn't read the whole thread, it sounded like you 
were saying something about the Qt-based (versus mythui) screens 
(including the Schema Wizard upgrade popups) not working properly on the 
Mac client, so it's quite possible that we always get the "Exit" 
response on the Mac client or something.
