I hope you used "Delete all video sources" and "Delete all capture 
cards" (not "Delete all capture cards on <hostname>") or the delete and 
re-create wouldn't help.

Start with a default-verbosity log of a mythfilldatabase run and, 
probably, a default-verbosity log of mythbackend startup and it will 
likely provide answers or--if nothing else--point us to a better set of 
logging options to use.
