In my case, Monk and Eureka are two examples that fail to lookup metadata.
In MythVideo I have them named "Monk" and "Eureka" and have filled in the
[series_name_override] section in ~/.mythtv/ttvdb.conf.

[series_name_override]
Monk:78490
Eureka:79334
