Spot on. I knew it would be something simple that I'd overlooked.

Once I saw your message I moved the database onto its own xfs and then
ext3 partitions without luck - I might need to do some more digging on
that front.

I'm temporarily mounting my ext4 partition with the barrier=0 option
and it has fixed the problem. Just need to investigate the long term
consequences of that option and power failures etc.
